In cases of massive bleeding, the first minutes are crucial for saving a victim. SICH first-aid instructor Oleh Leliakin explained how to properly act before medics arrive and apply a tourniquet.

Massive bleeding is one of the most dangerous threats following a severe injury and can lead to death within minutes. In modern first-aid protocols, stopping critical blood loss is a priority, with the tourniquet serving as one of the most effective stabilization tools.
Before providing aid, it is necessary to assess the situation and ensure the threat has passed or can be minimized. If the source of bleeding is clearly visible, the first step is to apply maximum direct pressure to the wound.
Action algorithm in a critical situation
If possible, apply a pressure bandage afterward and evaluate whether the bleeding has stopped. If not, a tourniquet must be applied five to seven centimeters above the wound, avoiding elbow and knee joints.
If the injury location is hidden under clothing, the tourniquet is placed as high as possible on the limb without wasting time searching for the wound. If bleeding continues despite a properly tightened tourniquet, applying a second tourniquet above the first is permitted.
Prohibitions and time tracking
In areas where a tourniquet cannot be applied, bleeding is stopped by tightly packing the wound with a hemostatic bandage and maintaining continuous direct pressure. Experts emphasize that a tourniquet must never be independently loosened or removed until medics arrive.
